Home Care Packages (HCP)

These services are a great fit for individuals that need tailored support for their lifestyle and well being. HCP is a government-funded program under the government’s Consumer Directed Care (CDC) approach.

 

When you choose our HCP you get greater flexibility and autonomy over your care and how our services are delivered to you. Eligible individuals will undergo a detailed assessment and be assigned a dedicated care coordinator to make sure they are getting the services they need in the time and manner that they have requested.

NDIS- Disability Care Services

We help eligible individuals receive the in home care they need so they can continue to live independently. Each client receives personalized in-home care focused on the client’s needs while prioritizing their independence, safety, and autonomy.

 

This is a good fit for individuals receiving government subsidized care. Fees are generally set by NDIS. Special cultural and linguistic needs can be met by our team so clients and their families can be comfortable while still getting the care they need.

Commonwealth Home Support Programme

CHSP is the new name for the Commonwealth Government programme formerly known as Home and Community Care (HACC). It helps aged people who need additional support to continue to live independently in their homes. Typically, support is limited to entry-level services such as light housework and personal care.

 

We offer services tailored to client’s physical needs, cultural and language needs, and client’s desires to remain independent in their community.

Private Care

All of our services are available to anyone, even if they do not qualify for any government-subsidized care. Care may be paid for with private funds.

 

Clients receiving government subsidized care can also purchase additional services not available through their government plan with private funds.

 

These services can be delivered on an ongoing basis or only for a limited time. The services are delivered per the instructions of the client and are designed to empower independence.
